I carry mine with a round chambered and the hammer at half cock. My main concern has been with the hammer getting snagged and falling with enough force to drive the firing pin all the way forward. If it indeed does have a firing pin block, then I'll start carrying with the hammer down.

I'd either politely refuse to answer his questions or turn it around and start asking HIM questions that go beyond the doctor/patient relationship that make him uncomfortable. Hopefully he would get the idea and stop asking others as well.

Like others have said, they're still a potential threat. My goal is to maintain my distance until law enforcement and the paramedics get there. To that end, I feel my only responsibility would be to call 911 and ask them to send help.
