How can you have faith in something that blew up in your wife's hands? Unless you were shooting reloads then I'd say there was something wrong with the gun. 1,000 rounds isn't enough to dub it 100% flawless in my opinion.
I had a Fobus for my Taurus PT111 and liked it except for the fact that it stuck out pretty far and was hard to conceal. If you're open carrying then it's not an issue. I'm not sure if the studs are an issue, I returned it because it didn't conceal well. Can you mail order something else?
My thought is that if the slide doesn't go all the way forward then the firing pin won't reach the primer. If her wrist is absorbing the recoil instead of the recoil spring then there's less spring force to close the slide.
