Ok, here is update. Everything that been advised did not resolved the issue. I sent gun to Beretta. And that is is what they found. "Found barrel chamber to be bad. Replaced barrel with shop overage. Test fired 20 rounds Firearm functions with factory parameters." I test as well. Issue resolved.

I have Beretta 92X, and never had any problems. Shot all kind of ammo with different magazines capacity from 10 to 30. Looks like you got the lemon.

No, I did not fire it yet, I just tried to cycle, as I usually do with a new gun.

The first round loads fine and extracts fine, but then the second round goes in the chamber, and the third comes after.
