Does it happen consistently every time with the same magazine? Every time with *all* your magazines? or just that once? If it was just once and didn't happen again, I would call it an anomaly, probably caused by your grip. Keep an eye out if it happens again, and mark that magazine.

+1 Uncle Mike's makes good cheap mag pouches and belts, but your weapon is only as good as it's holster. You should invest in a good one. Blade-Tech or Blackhawk, IMHO. Sorry I didn't specifically answer your question.
