1911 magazine question

I've noticed several "things" while shooting the gun "dirty". Easy way to tell what's impacting what.

Feed ramp "marks" clearly below where the bullet nose hits the ramp to feed on this reliable feeding 1911. I don't recall seeing those before, but could have missed them.
In addition, while checking ammo length, feeding, etc I've noticed on some mags, didn't test them all, that if I load using the slide stop, remove the mag the cartridge has moved forward in the mag.

Do you think after feeding the top round, the next round is moving forward and hitting the ramp? Mag "lips" too wide? I assume this is not desirable although no malfunctions as of yet anyway. Although, I can induce a malfunction by reloading the mag without pushing the round back.

After the first shot the rounds in the magazine are ALL moved from where they started. The top round in the mag should not be hitting the feed ramp, if it was it would be felt when pulling the mag out of the gun. What you describe sounds totally normal to me.

Exactly what is the malfunction you can get if you just pop the mag back in with the round slightly forward?

The nose of the SWC bullet jammed right into the barrel ramp on reload...hard!

And, a shot wasn't fired. What happened was I had a round with a reversed primer so just a "pstttt". Released the mag into hand(didn't drop it), removed the dud round, load mag in gun(noticed round was forward), release slide...jam. So, it "appears" the round moved forward when I intially loaded the full mag prior to the misfire?
Not enough to hinder the mag removal, but perhaps "long" enough to simulate a "too long" a round?
