I requested a C&R kit as described here, but when I received the packet, all that was in it was a blue fingerprint card (form FD-258), a distribution center invoice (listing "FFL Application Fingerprint Card") and a requisition form to request other ATF forms (ATF form 1370.2). Did they sent me the wrong thing, or is this the complete kit for a C&R FFL? I can scan and attach the form if it would help, but it doesn't even list whether I'm applying for a type 01 or 03 FFL.

Just call your local ATFE place, they will be able to hook you up.
That is all I did,and they sent me everything needed to apply.
You do know that once you get your C&R you will go broke!!!!

That sound slike the wrong application... I didn't have a fingerprint card or anything. IT should be a single page, duplicate(one for ATF one for CLEO). Make sure it is for an "03" FFL...

Go right to the BATFE web site and order the forms directly. They'll send you all you need.
