Evey now and then I would get a cap that did not go off.When this would happen, I suspected it may be a nipple or gun issue. But here lately I have been inspecting every cap that I put on.

The last several tins have had at least one or two that the green primer had fallen out.

It is easy to miss these when loading up.
So I have been slowing down to make sure that each cylinder fires,

I've had this same problem when loading my rifle. Snap! Nothing... empty cap. I even suspect that some never had primer in them to begin with...
 
I’ve had CCI musket caps do the same thing.The priming discs we’re laying in the tin.Bad thing was they were the same color as the inside of the cap.Hard to see which caps they were out of.
 
I have occasionally run into the same thing. I normally use Remington caps and just ever so often I will find a cap with no priming compound but the the little green "disc" will be in the tin. Somewhat related, I have also lost the green compound seating the cap on the nipple a bit too hard. During Cowboy shoots things can get a little hectic loading the percussion pistols and I gave the caps an extra hard push with with the seating stick to be sure they were on there. Had a bunch of miss-fires. Went to the unloading table, removed the caps and there sat the little green "discs" stuck to the inside mouth of the nipples. Mea Culpa on that one!
