I Use a Combination
The foam plugs that I find work best for me are the Howard Leight "Leight Sleepers" plugs (32 db), the striped red and yellow ones. Once they're in there's no sensation at all of having anything in the ear, and they allow me to sleep comfortably even in very noisy conditions.
When shooting, I wear them under a pair of cheap plastic-and-foam muffs, and the combination seems to work very well for me.
