How much practice w/ weapon mounted light?

For those of you that have weapon mounted lights, how actual practice w/ that set up do you actually do? Do you shoot w/ the light mounted? On or off? How crucial is it for the light to either have a pressure switch or on/off switch?
Specifically, a USP 45 w/ SF Nitrolon
