How would this thing be held?

I assume that the pointer finger would hold the gun and the middle finger be used to pull the trigger. I’m not really sure though, and I wonder if it was just so awkward to doom the design from the start.

Is the hammer the trigger? Just pull it back and release it?

That's what I think too. It's a belly gun, so a traditional hold and aim doesn't help much here and the ergonomics this forces you to would work fine when shooting from the hip.
 
-Looks like a Colt's No. 1 Derringer in .41 rimfire.
An awkward design that's pretty much a contact-range gun.
I tried and failed to hit a paper plate at ten feet.
-And yes, the pointer finger pulls the trigger, but the barrel winds up almost perpendicular to your fingers.
 
Looks like a cane gun, by the tapering, but could be a waistcoat gun like a knuckle duster

I have seen a similar design that was also intended to be used as a "knuckle duster".

I seem to recall seeing it called exactly that in an article I read about small single shot pistols.
 
Trigger finger on the trigger, but with a very “high wrist” position like holding a golf club, or pointing with a foil.
 
I don't know how you would hold that, but it would probably still be more intuitive than the USFA "Zip 22".
 
??? Looks like it could hook over the ear...naw,...maybe it's a custom made prosthetic gun for someone who's missing some of their parts.
