Would never happen on a Lee Pro 1000 with a bullet feeder attachment. (Primer might be missing or upside down, however).

You did nothing wrong. Solar flares cause misaligned bullets. Aluminum foil wrapped over the seating die will alleviate this problem. 60% of the time, it works every time.
