While firing at the range last thursday, my friends 1911 firing pin stop fell out of his pistol. Anyone know how that could happen? Ive been shooting mine for years and never seen this. We put it back in but he's worried there is a problem.
 
A loose firing pin stop combined with a weak firing pin spring might let it drop.
A firing pin sticking forward due to burrs or dirt can, too.

I would clean out the firing pin channel and replace the firing pin spring at a minimum.
 
Peening the edges of the firing pin stop used to be a standard item on the "to-do" list for 1911's.

Replace springs as Jim suggested and peen the edges of the firing pin stop if you want the extra assurance.
 
Replace the recoil spring and the firing pin spring (extra power firing pin spring) and definitely consider buying and carefully fitting an EGW oversized firing pin stop, it will keep the extractor from rotating and make ejection more consistent. When the firing pin stop falls out it means your firing pin spring is toast and the firing pin and extractor tunnel probably could use a good cleaning while you're in there. Replacing all of the springs in a 1911 every few years is a good idea.
 
The firing pin had to stick forward for the stop to fall out. All the suggestions should work at making it more reliable but the root cause is likely a dirty pin channel so a good cleaning of it is in order.
 
I had this happen on my RIA CS Officers at about round 250, Mine stopped at half way down so i didn't have to go looking for my firing pin and spring behind me at the indoor range like an idiot.
I can tell you mines channel was clean as i clean it every time manually taking it apart since its a series 70 and easy to do.
I bought a new heavier 22.5lb recoil spring from Wolff with a new HD firing pin spring.
I also ordered a new EGW FP stop and put just a slight radius on it as i use these on all my 1911's when i ever get the chance to swap them out.

Between those 2 items the gun not only is safer since i do ccw with it and dont want a MIA firing pin, But it also shoots alot better as the EGW stop actually softens the initial recoil which is kind of high in a 3.5" 1911.
