Have you checked the brass after firing to see if there are any signs of something wrong with the chamber?
I have looked at the brass and it looks good. the only marks on it are on the neck and they line up with the locking lugs (or what ever the official name for them is) on the barrel
 
gonna have 2 mic the inside dia. of the gas key...small set of calipers maybee.? as far the gas tube just remove it and mic it where it mates in the gas key.
 
Strange as it may seem, a too tight mag catch can cause odd problems. Try loosening it up a turn or two and see what happens. It's worth a try.
 
Could any of yall measure how many pounds it takes to pull back the charging handle. I just wonder if my buffer spring is too strong. Just sitting here thinking and I have no ar's to compare to
just got around to measuring it. showed 14-16 pounds most of the way back
 
if you take the gas key off...35-40 inch pounds to tighten it. I dont think thats the problem but i could be wrong. try to mimic the operation by opening the upper and sticking a finger in the back of the carrier and work it in and out to see if the carrier is binding or the bolt is having trouble unlocking. Have you checked the headspace?
