We had a Cowboy Action Match at dude ranch. Static firing line, 6 stages, every stage hot and shooting. A dude wrangler took about 15 kids on horseback directly across the range, about 400 yards from the firing line. Lots of puckering until we got things shut down,which took a while because not everyone saw the idiot immediately.
 
I would say it is extremely rare. Any time this happens at a major especially, you are going to hear about it online these days, whether there is video or not.

Just in USPSA, I count 57 majors scored on the website for 2014. If those average 220 shooters and 15 stages, that is 188,100 stage starts. If one of those starts with someone down range, that is over 99.999% of stage starts without someone down range. Considering all the other sports and club matches at which this never happens, I would say the safety record is actually much better than that.

Overall, USPSA's safety record is impeccable.

Probably not a big deal because Cowboy action ammo won't travel 400 yards, right? ;)

I hear you. About like black powder/muzzleloaders won't travel far either...
