I get pages 1, 2, and 3. Print double sided, etc. But pages 4, 5, and 6 duplicate pages 1-3.
Does filling out pages 4, 5, and 6 constitute the 'submit in duplicate' part? Or do you fill out 1-6, then print another copy and fill out 1-6 again?
If 4-6 are the 'duplicate, does it matter that 4 and 5 are on different sheets of paper (they are the duplicates of 1 and 2, that must be on either side of a single sheet).
