After 4-500 trouble free rounds, I have started getting the issue above. It seems to happen about once in a 50 rd box. Everything feeds slightly to the left, and sometimes they feed all the way to the left and hit the left side of the chamber. When this happens, the rear of the round is still in the mag, the old case ejected, and the slide rests against the side of the jammed case.

I put some strips in next to the mag to be sure it wasn't twisting, but nothing changed. I believe I tried another mag, but may have just dreamed it. The only thing I could think of otherwise is not enough crimp on my cartridges causing them to sit funny in the mag. I started crimping more, but haven't tested yet.

Any thoughts?

Try another mag for sure, when you are wide awake.

That is the most likely cause, and always the first thing to rule out.

How does your reloads look? What bullet are you using? Have you checked to make sure that the bullet is seating squarely in the brass and not off set. What I mean is look at the loaded round and check if the brass buldges on one side. If the rounds will drop in the chamber and you removed the flare, it shouldnt be a crimo issue.

Well, I took a box of reloads out, and had no problems. I had crimped those ones a bit more, and figured I would go to the other mag if I had issues, but so far so good. I can see from the shiny spot in the soot on the feed ramp that everything is feeding to center now, so am hopeful things will be good from here on out. I was trying not to overdo the taper crimp, and I think I just kept using less and less, until there was a bit of flare left that kept the cartridges from feeding straight in. Will know better after another couple boxes to be sure.

Use your dial calipers and measure the case mouth of a loaded round.

Your taper-crimp should measure .421".
More then that needs more crimp.
Less then that needs less crimp.
