I voted yes. It was a reloaded shot shell. I was trying to reload some cheap hulls and they would not hold a crimp so not enough wad pressure to get full burn on the powder
 
I've had several. All of them have been my own reloads and generally fall into a category of loads.

I (used to) shoot Bullseye. I cannot find a local match these days. I have a .45 ACP 'wadcutter' pistol. While experimenting with ultra light loads, some where too light. I had a similar problem with a .38 Special PPC revolver.

I was fire-forming some brass (.30-06 to .30-06 Scott's Improved) and one bullet stopped with the nose exactly at the end of the muzzle.

Nothing too serious, but annoying.

Cannot remember any factory squibs. Had some problems with .22 rimfire ammunition not firing at all.
 
Never with any centerfire round, but plenty with .22 rimfire, especially CCI products. Just today a squirrel needed a second head shot because of an underpowered CCI round. Those underpowered ones don't quite hit where I'm aiming.
 
I had a non-fire with a Remington factory round of 25-06 back in 1991. First time I had ever fired a shot at a buck and the hammer on the borrowed 98 Mauser went "CLICK"---twice. I ejected it and the next one fired, so all's well that ends well.
