I picked up a pair of the old GI suspenders for the wet weather gear today they seem to be working out
 
Yes a good belt is nice, and works somewhat, but not completely if you have the wrong shape. Suspenders are a life saver. They prevent pants from falling, thereby keep the belt in place, and therefore keep the gun in place. Suspenders also help support the back and the hips more evenly when you are carrying and make carry all the more comfortable. SS agents use em frequently - long days on their feet carrying.

I'm in the same boat as the OP (noassatall disease). I used a wide and relatively thick leather belt. On top of that a quality holster will help out quite a bit. Still starts to slide down every once in a while, but when it does happen I make a trip to the bathroom or somewhere I can discreetly adjust it. During winter I carry in a shoulder rig underneath my coat.
FWIW I carry a Glock 17. Not very heavy, but its no little guy either.
 
A one-inch leather belt? That is a good part, if not all of, your problem.

I am quite fond of the Wilderness Instructor Belts that I have. They are very stiff and do not bend in the middle. The Velcro on the outside anchors the loose end of the belt and will catch on at least one loop of your holster, helping to cinch it in place. And the adjustability is infinite, unlike a leather belt where you are limited to one or the other of the buckle holes.

A high quality leather belt, if it goes better with your clothing or if you just prefer leather, is made by Jim Speidel, the Beltman. Great belt, but it has the aforementioned adjustability problem. Sometimes one hole leaves things too loose, while the next one can be too tight. He does offer an option of seven holes 3/4 of an inch apart, instead of the usual 5 holes/1 inch apart.

His belts are excellent. I recommend a 1.5" belt. I've had a 42oz N-frame on and not had any difficulty keeping my pants on. I too suffer from "smallarse-itis".
 
A wider belt of good gunleather would help. Also, I not sure what brand of holster you use, but some IWB holsters are better than others at distributing gun weight.

It sits wherever you put it. I wear mine about where an IWB would be and it never moves unless I'm all-out sprinting. No harder to draw from than an IWB IMHO. The only way I could see it being harder to draw from is because it holds the gun closer to your body than many holsters, but that's a good thing for concealability.
 
A 1" belt is tiny, as far as gun belts go. 1.25 is typically considered the minimum, 1.5 is a good middle choice, 1.75 or larger for best carry for heavy guns.
The 1.5" is about as big as typical pants loops go. A few go larger... but mine are pretty much all 1.5". Some of the 'tactical' pants and some blue jeans might go larger.

Personally, I hate carry on the belt. Shoulder carry is easier for me... but if you do belt carry, get the biggest, sturdiest belt your pants can handle.
 
Yep, same problem here.
I went from H&K P2000 (a heavy a$$ gun) to Glock 19...
Now I'm considering a G26, or out right stealing my wife's KelTec P32 :evil:

But in all seriousness, I think I'll try a shoulder holster. No matter what gun I put on my belt, it pulls down. Even my S&W 442 in OWB holster.

And yea, I'm 6'4" 175lbs... no assusminimus here too.
