Got my forms to fill out and send in to turn a Draco into a SBR.Am confused on how to answer some of the questions.Is there a sample form I can find to look at or a forum to go to that would help me to fill out the forms?

The form 1's are pretty self explanitory... you will need to make sure you submit the following forms if you are an individual and living in an SBR friendly state:
ATF F 5320.1 (Form 1) — Application to Make and Register a Firearm
IN Duplicate, if you print, make sure front/back are on same piece of paper, otherwise ATF will send back to you
ATF F 5330.20 — Certification of Compliance with 18 U.S.C. 922(g)(5)(B)
You only need to send 1 of these (if you don't ATF will wait a few months, then tell you that you are missing it)
2 - Fingerprint Cards -(Your local police/sheriff's department will do them for you for $10)
2 - Photos
CLEO Signature - Sheriff in your area, DA, or Judge
1 - Money Order for $200.00 (you can send checks, but they just add time to your wait)
These forms are FREE from ATF, you can use the PDF's or get hardcopies free from them.

In my state LA we have another form you have to get the State Police to Approve
Application To Register Firearm - LA Revised Statutes, Title 40, Section 1783

In Louisiana this form has to be approved, prior to sending in to the ATF.
If you own an LLC or start a TRUST then you can skip the fingerprints, photos, and LE signoff...
