OOB or Over Pressure?

Picked this up on the range. Not my round but in all the years of reloading I've never experienced one of these. Out of battery or over pressure?

I can only guess but the way the rim looks like it has been subject to a lot of pressure.
The case head has expanded to where the primer has fell out.
 
No matter the pressure the barrel wouldn't let that much of the case bulge unless it blew. The brass can only swell to the size of the chamber assuming it's actually in the chamber. And if the chamber blows the brass blows too. I'm guessing it had to be out of battery of it got that way by firing. But I've never seen anything exactly like it.

I've never had that fiocci brass but have read a lot of bad comments about it. It sure looks to me like that brass withstood more than it was supposed to. Either way, someone came close.....

or they screwed up reloading and threw the brass out of a pile onto the range maybe in thier baggy of ammo? The lack of black around the blown primer has me wondering if it hasn't been through a resizing die and the rim been bent by the shellholder after it stuck.
 
I looks like it has a gigantic primer flash hole. I wonder if it was drilled out for wax loads and made back into his normal shooting brass?
May just be the picture.
