I have not yet loaded a squib, or had a primer I seated fail to fire.
I did have a .357Mag round stick a bullet in the barrel, but it was one of 2 rounds my "Mentor" at the time gave me and said 'here try these' The first one made a funny noise, I cranked back the hammer (Ruger Blackhawk) on the second one and my brain interrupt said 'that didn't go right'. I gently lowered the hammer and unloaded it, and pulled the cylinder to check the barrel, There was a bullet stuck in the barrel. Sadly I did not have any way to push it out so I had to go home. Pulled the bullet on the other round, NO POWDER! That was the first and last time I trusted his reloads.

Yes, one. I added an overhead lamp to be able to visually check every round.

I use a Lee ALBP, and to be honest, it is rickety enough to cause a lot of drama. In the supply crunch I have been building the character of my juvenile slaves by having size/decap separately on my hand press. This eliminated 90% of the drama, and the rest of the processes run far easier now.

I told my boys that one of the reasons for having kids is so I would not have pick up dog poop or mow the yard

Mine were told the same thing, but with the addition of "so I wouldn't have to pick up brass". It was a sad day the first time I went shooting after the last one left home. I stood there looking at a pile of 32 ACP and wondering when it was going to pick itself up. I've since rekindled my love affair with the revolver! LOL.

Well, the good news is, if it fired and cleared the barrel, it isn't a squib load. My understanding of the word has always been that a squib is an underpowered load which does not clear the chamber/barrel.

I am glad you posted that. I thought I once (factory ammo) had a squib but the revolver unloaded all empty casings. But it did not fire on one trigger pull, so maybe it was almost a squib and actually cleared the barrel, or maybe it was a hangfire because I remember starting to bring the gun down and it fired but I may have pulled the trigger, cant remember. Not having the understanding at the time, had it been a squib I would have grenaded the gun because I kept firing. Now I know better.

Good thing now I know about hangfire so next time it does not go bang, I can wait the 30 seconds in case of hangfire, then inspect the gun--always pointing it downrange just in case.
