    As I recall you need to provide the usual name, address, phone#, etc. plus drivers licence number. You also need to agree for them to run a background check(and pay $50 for that). Finally, they require registration 2 weeks prior to the class. The sign-up form and class info is on their webpage.

    I have taken several classes at FS and would say it was definately "worth it" for me. The "worth it" measure depends on where you are coming from - both in terms of training and ability as well as geographical location. It also depends on if you are using time or money as your "worth it"...
