Cylinder Spinning After Closed

I had an incident today at the range that is concerning me. After ejecting my empty cartridges and reloading, I shut the cylinder...usually I spin it a little and it locks up. Today it just kept spinning. I pushed on it to make sure it was locked in place and it was. The thing is that this only happened one time out of about 30 reloads. Any idea what is causing this and if the revolver needs to go back to S&W?
 
Sounds like some crap floating around inside the gun got stuck beside the locking bolt.Or, the little coil spring tunnel in the bolt is full of dried grease & dirt.

I would suggest you remove the side-plate and give the internals a good cleaning & lube job if you feel confident to do it.

If not, then hose it out good with WD-40, Gun-Scrubber, etc, and blow it out with an air compressor.
Then lube liberally with a light gun oil and then blow out the excess oil.
