Form 1 Help

Hey guys, I ordered a Form 1 and fingerprint cards from the ATF so I can shorten a shotgun and I've got a couple of questions about it. I'm cutting down an old Ithaca SxS, and while I've submitted plenty of Form 4s, I've never actually done a Form 1.

First, they sent me a 922 form as well. Do I need to fill this out and send it in with the Form 1? I don't have a problem doing it, I was just curious if it was necessary.

Second, there is a box asking if the gun is being reactivated - from the way I understood the directions, i thought it was asking if this was a previously registered NFA item that was deactivated somehow and was being returned to service, so I put no. Is this right or did I screw it up?

Yes, turn in a 922 form.

Reactivated is DEWAT turning into a REWAT. Not applicable.
