Deep Pockets
I have found that pleated with a loose cut do best. The biggest issue I have found is finding pants with deep enough pockets.

I like the Columbia Lander pants. It has that one darn pocket on the side which if left empty doesn't stand out too much. Much better than the typical tactical, cargos which are fine for after wear or the weekends. Nothing beats the Lander with the pocket behind the pocket with a zipper. No elastic in the waistband fortunately. You could remove the zipper string, and smooth the zipper tongue into the seam and you can't tell there is another pocket. The pocket is nicely sized for a X42 in a Mika and since it is behind the regular pocket when you add keys or a cell phone it breaks the virtually non existant print.

Fortunately you can still find plenty of Dockers, Izod, and Savanes with pleats that do tend to conceal better.
I have actually thought about having my tailor add/sew in a couple inches of pocket to my strong side pocket. Slightly rounded of course for the Mika Round cut. If I were going to carry with the Hogue Monogrip it would be a must if the shirt is tucked.
Some people have gone to ankle carry for the very reasons you have toppled to. Jackets with interior pockets work nicely. You may want to check out the Scottevest Products, but again they are more for professional packers or weekend wear.
