The report about the ban on SERPA holsters is actually made available by FLETC. Someone made it available under FOIA, and once it was thus cleared, FLETC themselves published it. Anyone who's concerned about shooting oneself with SERPA can download it and read it.

My take-away:
- the hazard is real, although very unlikely; before FLETC banned SERPA, only 4 discharges happened
- it was surprising that SERPA is dangerous when drawing, not holstering like most other cases
- SERPA is not the only design that features an unlock with the trigger finger
