I also use a Milt Sparks VMII IWB for my Kimber 4" 1911. Never does the thumb safety disengage without me manually flicking it. I've always felt safe with a cock and locked 1911 (because i train regularly and shoot IDPA weekly with it). I know it will not accidentally shoot (i understand the internal mechanism of my gun) and make sure the thumb safety and grip safety always works everytime i clean my weapons.
 
I've been very happy with my Andrews MacDaniel II IWB. Excellent quality and very comfortable. I switched out the ambi safety on my TRP for a single sided. No problems either way.
 
I have carried a full sized 1911 in a Ross Leather Model 18 and a Kimber Compact Stainless II in the Ross Model 18 and a Milt Sparks Summer Special 2, both in condition one. I have once found the Kimber inexplicably in condition zero at the end of a day in the SS2. It is not something I worry about, given the grip safety and the firing pin safety.
 
I have knocked my ambi-safety off on a Springfield EMP before; for this reason any 1911 I would carry will have only a right side safety, and a holster with a sweatguard covering the paddle.
 
Kramer IWB #2 with 5" Colt Government, Lightweight Commander and Browning Hi-Power all with single sided safeties only without the safety ever disengaging.
