I've had good luck with Lobogunleather, and good value from Ray's Holsters (good basic clamshells for j-frame). Also Andrewsleather makes good heavy duty holsters.

FIST ultra-thin Kydex for me

Custom made by me for me - leather/Kydex hybrid, left-hand IWB. Similar to those made by and shown in rustygray's post. I use some beige Kydex that I got surplus locally. Cost me less than 10 bucks for a full 4' by 8' sheet of the stuff. It was originally to be used in aircraft interiors. Not marked with the Kydex brand, but has the same thermoplastic properties and forms just like it.
