Case rim bent in two places? Bullet puller?

I have some once-fired (by me, in my rifle) Hornady/Frontier brass and my cartridge OAL went nuts on me. On closer inspection, on several of my cases the rim is bent in two places about 180 degrees apart.

All my other cases with the same prep are fine. The only differences are that I primed, charged, and seated these particular ones at a different time (although with less force then the good batch.)

The only other variable I can come up with is that they were damaged in my bullet puller. I use a Frankford Arsenal inertia puller w/ a regular shellholder instead of the goofy collet.

Has anyone ever seen anything like this before?
 
My canera broke so I can't post a pic. It appears as though it would be the extractor peeling the case head back away from the rest of the case except there are no extractor marks and the damage is in two places opposite each other.

I'll try to come up with a camera if possible.
 
It might be the bullet puller. They use the collet to maxmize the bearing surface. A standard shell holder would only hold it on opposite sides as you describe as it's free to move back and forth in the holder.

Even though it's a PITA I recommend sticking with the correctly sized collets.
 
Sounds like the puller to me. That is the kind of damage you see when you get a case stuck in a die and the shell holder bends bends the rim, then tears it off.
