If a sidearm is worn it should be carried in condition 1, meaning that if it is pulled from the holster for a defensive purpose it is ready to go. In most defensive situations a person doesn't have the time or motor skills to fumble with a safety or a hammer.

That said, all of my holsters properly cover the trigger.

In the 1911, Condition 1 is round in chamber, hammer cocked, safety on.
