The extractor was the problem. After putting a little more tension on it the problem went away. I also learned that too much bend will cause FTF. Taurus is sending me another extractor.

My Colt (new) S70 did that as well, though not as bad. As others have said, I believe it's an extractor tuning issue. I never got around to adjusting the tension before I sold it, but it ran reliably enough for me.
